The power of talk and power in talk: a systematic review of Indigenous narratives of culturally safe healthcare communication

Abstract

The study aimed to explore Indigenous narrative accounts of healthcare access within qualitative research papers, to better understand Indigenous views on culturally safe healthcare and health communication represented in that literature.
A systematic literature review of peer-reviewed academic qualitative studies identified 65 papers containing Indigenous respondents’ views on accessing healthcare.
Analysis included all Indigenous voice (primary quotations) and author findings describing healthcare access across these studies.
Healthcare communication, or ‘talk’, emerged as a key theme. Indigenous clients valued talk within healthcare interactions; it was essential to their experience of care, having the power to foster relationships of trust, strengthen engagement and produce positive outcomes.
By mediating the power differentials between health professionals and Indigenous clients, talk could either reinforce powerlessness, through judgmental down-talk, medical jargon or withholding of talk, or empower patients with good talk, delivered on the client’s level.
Good talk is a critical ingredient to improving Indigenous accessibility and engagement with healthcare services, having the ability to minimise the power differentials between Indigenous clients and the healthcare system.

The AH&MRC has developed a new vibrant Sexual and Reproductive Health Resource Kit for workers to use with Aboriginal young people named “DOIN ‘IT’ RIGHT!”.

DOIN IT RIGHT! provides workers who work with young Aboriginal people (including non-sexual health and non-Aboriginal workers) with step by step instructions on delivering sexual and reproductive health activities appropriately.

Although the statistics are sobering, ongoing education and health promotion will assist young Aboriginal people to make informed choices about their sexual and reproductive health. Given the decreasing age of first sexual experience, high rates of STIs and teen pregnancy, it is important that age and culturally appropriate information and education is provided to young people from an early age.

Documentary gives insight into risks of sexual assault among Australia’s international students

Half a million international students, most from Asia, are enrolled to study in Australia. It’s the country’s third largest export industry, worth $18 billion.

But Australia’s reputation as a safe and sunny place to study is under threat after widespread disclosures of rape and sexual assault.

Australia: Rape on Campus follows a six-month investigation into sexual assault at the country’s universities, exploring how international students, far from home and family, are especially at risk.

It follows an Australian Human Rights Commission survey which found 1.6 percent of students experienced sexual assault in a university setting in 2015 or 2016, one in five were international students.

Journalist Aela Callan is behind the documentary and she spoke to ABC’s Fiona Ellis-Jones from Berlin.

Her documentary, Australia: Rape on Campus, will be screened on Al Jazeera.

Pregnant women are at increased risk of domestic violence in all cultural groups

Domestic violence occurs across all age groups and life stages. Rather than reducing during pregnancy, expecting a child is a key risk factor for domestic violence beginning or escalating.

Our research, published today in the journal BMJ Open, found that 4.3% of pregnant women due to give birth in Western Sydney disclosed domestic violence when asked about it by a midwife at her first hospital visit. The study examined more than 33,000 ethnically diverse women who gave birth between 2006 and 2016, and found that these disclosures spanned all cultural groups.

‘People are scared’: the fight against a deadly virus no one has heard of

An Aboriginal woman – we’ll call her B – is sitting in a dry creek bed outside her community and telling the world “this is a very bad disease. But we have to talk in a way not to shame people. Not telling them straight out. Telling them gently and quietly.”

B is talking about a sickness that has killed her family member and is a potential tragedy facing Aboriginal communities in central Australia, who have the world’s highest rates of a fatal, human immune virus for which there is no current cure, no treatment and no coordinated public health response.

Human T-lymphotropic virus type 1 (HTLV-1) is transmitted through sexual contact, blood transfusion and from mother to child by breastfeeding. It can cause a rapidly fatal form of leukaemia. Some people die within weeks of diagnosis. HTLV-1 also causes inflammation of the spinal cord leading to paralysis, severe lung disease known as bronchiectasis and other inflammatory disease.

In five communities around Alice Springs, more than 45% of adults tested have the virus, a rate thousands of times higher than for non-Indigenous Australians.
